Established in 2010, this hospice program was certified by the state of Texas and accredited by CHAPS in 2011.

Since inception, the level of professionalism and quality of care has resulted in consistent growth, solidifying a position of leadership in the community for which it serves.

There are currently 152 patients on census with the anticipation of 160-170 by the end of 2016. Averaging 50-60 new admissions per month.

The organization is licensed to provide services throughout Region 6, as designated by the Department of Aging and Disability Services (DADS). The service area is comprised of the following thirteen (13) Texas counties. They include: Austin - Brazoria - Chambers - Colorado - Fort Bend - Galveston - Harris - Liberty - Matagorda - Montgomery - Walker - Waller - Wharton.

With over 75 years in combined experience, this organization is guided on a daily basis by one of the most experienced and tenured hospice management teams in the state of Texas. All members of the management team are willing to remain in place post close.

A preliminary review of all books and records indicate a clean provider, void of any previous, pending or preexisting legal, financial, tax, or Medicare/Medicaid liability, in any form.
